Gender and Sexuality Across the Lifespan and Access to Healthcare and Public Health Services Childhood abuse, including sexual, physical, or verbal abuse, can result in adult health issues. Approximately 5% of patients with a history of childhood abuse will report the abuse to a physician but most physicians report that they do not screen for abuse. On average, 20% of men and 25% of women report childhood abuse. (Springer, 2003) Childhood abuse has a consequence of psychological and physical health issues. Psychological ramifications may include depression, anxiety, eating disorders, PTSD, and aggression. Physical health issues may include chronic pain syndromes,fibromyalgia,chronic fatigue syndrome,and irritable bowel. Victims of childhood abuse are also more likely to engage in high-risk health behaviors including smoking,alcohol and drug use,and unsafe sex. (Springer, 2003) Childhood abuse is positively related to adult depression, aggression, hostility, anger, fear, anxiety disorders, and personality disorders. Men and women utilize health care services with different degrees of effectiveness. Women are more predisposed to utilize health care services than men. Studies have shown that there is a direct link between thoughts of weakness and rejecting help aspect of masculinity and help seeking behavior. By dismissing their health care needs, men are constructing gender. When a man brags, I haven’t been to a doctor in years, he is simultaneously describing a health practice and situating himself in a masculine arena. (Hunt, 2011) As a result of this masculine social construct, men may under utilize health care services, are less likely than women to seek medical care when they are ill and prefer to tough it out until they cannot handle it anymore. Men’s reluctance to seek health care is a significant barrier to improving their health. Their reluctance to seek care decreases their chance for early detection, treatment, and prevention of disease. Health professionals should move primary care into the workplace to make it easier for men to access health services. Women are more likely to seek health care services than men. The idea that women may seek services more readily and men will hesitate may result in health care providers assuming that women have a lower level of symptom tolerance before deciding to seek. This could affect a providers evaluation of the symptoms, their diagnosis, and their strategy for helping a patient manage their symptoms. Patient gender can influence a doctors’ proposed management strategy. For example, when doctors were shown videotapes of identical symptoms which could be indicative of coronary heart disease, female patients were asked fewer questions and were recommended for fewer diagnostic tests. Doctors were more likely to tune into psychological cues and to search for psychological explanations for symptoms when the presenting patient was female. Gender identification can be a determining factor to varying degrees of effectiveness to health. These can include how a particular gender deals with stress, environmental toxins, and poor nutrition or lifestyle choices. These factors can vary in women, men and any identification in between.
